What the HBOT studies actually show — senescence, telomeres, healing, and performance.

The foundation: U.S. Navy decompression sickness treatment protocols

The clinical basis for HBOT as a tissue repair and inflammation intervention was not established by wellness researchers — it was established by the U.S. Navy. Decompression sickness causes nitrogen bubble formation in tissue, producing localized inflammation, joint pain, and cellular injury. Recompression in a hyperbaric chamber forces nitrogen back into solution and floods damaged tissue with oxygen, resolving inflammation and accelerating repair. These protocols, developed over decades of military dive medicine research, are the same biological mechanism now being studied in longevity, recovery, and anti-aging applications. The U.S. Navy Dive Manual remains the foundational reference for hyperbaric oxygen as a clinical intervention.

Wound healing and tissue repair

Multiple randomized controlled trials have demonstrated that HBOT accelerates healing in chronic wounds, diabetic ulcers, and radiation-damaged tissue by increasing oxygen tension in hypoxic areas and stimulating angiogenesis. A Cochrane review of trials covering 147 diabetic foot ulcer patients showed a significant reduction in major amputation risk when HBOT was used as adjunctive therapy. Note: clinical wound healing studies use 2.0–2.4 ATA with 100% oxygen. Home chambers operate at 1.3 ATA with ambient air — a milder application of the same underlying mechanism.

Inflammation reduction

Under inflammatory conditions, HBOT has been shown to attenuate pro-inflammatory transcriptional signatures, reduce cytokine levels including IL-6 and TNF-α, and decrease markers of oxidative damage. Importantly, this anti-inflammatory effect is most pronounced in individuals who already have active inflammation — research in healthy volunteers without pre-existing inflammation shows minimal cytokine change. This is consistent with decompression sickness treatment: hyperbaric oxygen resolves inflammation in injured tissue specifically because the tissue is already in an inflammatory state. The mechanism is well established; the wellness application is an extension of that same biology.

Exercise-induced muscle injury and recovery

A 2025 meta-analysis of 10 studies covering 299 subjects found that HBOT significantly accelerated recovery from exercise-induced muscle injury across both higher and lower atmospheric pressures, with effects observed at both 60 and 100-minute session durations. It should be noted that the specific claim of HBOT reducing delayed onset muscle soreness (DOMS) in healthy individuals is not supported by the Cochrane evidence — that research was inconclusive. The recovery benefit applies to actual tissue injury and inflammatory states, consistent with the decompression sickness treatment model. Healthy muscle soreness without underlying injury is a different condition.

Anti-senescence and telomere lengthening

A 2020 peer-reviewed study from the Sagol Center for Hyperbaric Medicine and Research, published in the journal Aging, found that 60 daily HBOT sessions in healthy adults aged 64 and older produced significant increases in telomere length across multiple cell types — over 20% on average — alongside reductions in senescent cell accumulation of up to 37%. The authors described it as the first demonstration of a non-pharmacological, clinically available method for senescent cell clearance. Protocol used: 100% oxygen at 2 ATA for 90 minutes per session. Home chambers operate at 1.3 ATA with ambient air — this is a milder application, and home-specific equivalence data is still emerging.

Cognitive performance and neuroplasticity in aging populations

Research from the Sagol Center has examined HBOT's effect on age-related cognitive decline, showing improvements in attention, processing speed, and executive function in older adults alongside measurable increases in cerebral blood flow. The mechanism — increased oxygen delivery to hypoxic tissue — mirrors the same principle applied in dive medicine for hypoxic injury treatment.

I didn't get into hyperbarics through a wellness blog. I was trained as a commercial diver by the men who helped write the book — literally. The divers who trained me were among the human subjects the U.S. Navy used to develop the decompression treatment protocols that became the standard of care for hyperbaric medicine. They got bent. Their blood wrote the manuals. That's where this knowledge comes from.
